Certification in health-related field is preferred.
Ability to completehealth training programs.
Demonstrate awareness, sensitivity and appreciation of Native culture, traditions and socioeconomicneeds.
Current CPR and First Aid certifications.
Must have and maintain a valid driver' s license with clean driving record.
Must be able to lift at least 25 pounds and move about with ease.

RESPONSIBILITIES:
Work in conjunction with providers and clinic staff to improve the health status andhealth knowledge of the tribal community, focusing on tribal elders and children.
Visits clients in their hometo perform vital sign measurements, diabeticfoot exams(visual and sensory), diabetic blood testing, and perform minimal patient assessments.
Provide generalhealtheducation in the use of such things as: walkers, crutches,wheelchairsand canes;minimal nutritional educationandassistclients with referrals.
Provide non-emergency medical transportation for eligible clients.
Provide health promotion/disease preventionactivities in collaborationwith clinic staff.
Responsible for disbursement and monitoring of medical supplies and pharmaceuticals toclients.
Prepareand submit monthly, quarterlyandannual reports as required.
Identifyandmaintain clientdata base.
Attend necessary trainings to improve and enhance services.
Complete any other duties as assigned by the immediate/controlling supervisor.
